2.11.1  Demand structures

The Japanese healthcare system is known for its efficiency and comprehensive coverage. It offers the population access to a wide range of medical and dental services, including preventive measures. The central and state healthcare system (SHIS) provides healthcare for 98.3 % of the population. 58.6 % of the population have health insurance through their employment, 27 % through a special health insurance scheme for the unemployed, pensioners up to the age of 74 and the self-employed. All adults aged 75 and above (12.7 % of the population) are automatically covered by another health insurance scheme (SHIS) specifically for older people. The remaining 1.7 % are covered by a public social assistance programme. The system is mainly financed by taxes and compulsory contributions (84 %). For treatments covered by health insurance, the co-payment is 30 % for people under 70 and for those over 75 with an income equivalent to that of the working population, and 20 % for children under 6. This also applies to senior citizens aged 70 to74. Over 75-year-olds with a low income pay a reduced deductible of 10 %194 40 % of total dental expenditure is accounted for by the over-65 age group.195
