2.2.1  Demand structures

There is a statutory social insurance scheme with compulsory insurance up to an upper income limit (Bismarck model). The dual system of private (Private Krankenversicherung, PKV) and statutory health insurance (Gesetzliche Krankenversicherung, GKV) is unique in Europe. Around 10 % of the population are fully insured through private health insurance, while 22 % of those insured through statutory health insurance have private supplementary dental insurance. The number of supplementary dental insurance policies has increased precipitously in recent years, by 3.4 % in 2022 alone to currently 18.4 million.18 The state sets the framework conditions for healthcare, while self-government (all those involved in healthcare) is responsible for organizing care. Health insurance contributions, which are paid equally by employees and employers, flow into the health fund, from which the funds are allocated to the health insurance funds via the Morbi-RSA (morbidity-adjusted risk structure compensation). Insured persons have a free choice of (dental) doctor. Private households are the most important payer for dental care after the statutory health insurance system. Within the European context, Germany has the highest proportion of public funding. The regional associations of panel dentists (Kassenzahnärztliche Vereinigungen) are responsible for ensuring outpatient care. Statutory health insurance covers all costs
